Here are 3 questionnaires concerning your health that Olivier and his team can deploy in your company, wherever it operates. They are proposed here to be tested anonymously.

None of the data collected is used for marketing purposes afterwards.

Musculoskeletal survey

The musculoskeletal survey (or MSD survey) allows you to know who feels pain where, with what intensity, and to what extent the pain is (subjectively) associated with the work.

It allows you to prioritize your ergonomic programs.

The demographic questions (first part) are adaptable to your organization. A final section can also be added to define additional indicators.

Wellness survey

This questionnaire is based on the WHOQOL Bref, developed by the World Health Organization to measure quality of life.

In this short version (2 minutes per participant), we only measure your physical and psychological wellness.

Your scores will be sent by mail after you filled in the survey. We will never use your address for any other purpose than sending you the results.

Örebro questionnaire

This standardized questionnaire is a clinical tool. It is useful either for yourself or for your occupational health department.

It is a questionnaire that, if completed 4 to 12 weeks after a musculoskeletal injury, can predict the risk of long-term absenteeism, and thus enable rapid action to be taken.
